They execute "Craniotomies." If a patient is rushed in with a skull crushed in a motorcycle accident, the Neurosurgeon drills holes in the skull, removes the shattered bone, and sucks out the pooling blood to stop the brain from being crushed to death.
They perform "Spinal Fusions." If a patient has a shattered spine, the Surgeon cuts open the back, navigates around the impossibly fragile spinal cord, and drills massive titanium screws into the bone to bolt the spine back together, saving the patient from permanent paralysis.

2 to 4 YearsYou secure a placement in the Neurosurgery ward. You do the grueling grunt work: holding the retractors for 10 hours, shaving the patients' heads, drilling the emergency burr holes at 3 AM, and managing the post-op ICU patients under strict supervision.
3. Master of Neurosurgery
4 to 5 YearsThe absolute, brutal barrier to entry. You must be accepted into the highly competitive local Master's program (e.g., at USM or UM). You study hardcore neuro-anatomy and execute thousands of supervised skull and spine surgeries to earn your Specialist title.
4. Clinical Specialist (Pakar Bedah Neuro)
3 to 5 YearsYou are a recognized expert. You command your own operating theater. You are trusted to crack the skull and execute the emergency trauma surgeries entirely on your own.

Minimum Academic Reality Check
Undergraduate
Medical Degree (MBBS or MD) recognized by the Malaysian Medical Council (MMC).
Postgraduate
Master of Surgery (Neurosurgery) from a recognized local university or passing the grueling international FRCS (Neurosurgery) exams.
Licensing
Full Registration with the MMC and inclusion in the National Specialist Register (NSR) as a Neurosurgeon is the absolute legal mandate to cut open a skull independently in Malaysia.
